Job Summary

Home-Based Primary Care Nurse Practitioner delivers geriatric and palliative primary care within the HBPC program. Responsibilities include developing and implementing individualized plans for population health and disease management; leading and collaborating with an interdisciplinary team; ordering and interpreting labs and diagnostic studies; prescribing pharmacologic and non-pharmacologic interventions; initiating referrals to specialty services; educating patients and families; assessing and adjusting the care plan to optimize outcomes; providing care in the home of Veterans with complex needs (including Health Promotion, Health Prevention, and telehealth/VVC visits); making home visits and telephone visits; exemplifying ICARE values; and delivering competent care to diverse Veterans populations. Preferred experience includes 2-3 years NP experience with geriatrics, high-risk patients.
