Home-Based Primary Care Nurse Practitioner

The Nurse Practitioner provides advanced clinical practice within the Geriatrics and Extended Care Service, delivering home-based primary care to Veterans. Responsibilities include developing individualized plans to optimize population health and disease management, leading and collaborating with the interdisciplinary team, ordering/interpreting labs and diagnostic studies, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals to specialty services, educating patients and families, assessing and adjusting care plans to achieve optimal outcomes, and delivering care in the HBPC program including health promotion, geriatric and primary/palliative care. Care is provided in the homes of Veterans with complex needs, including VVC and telephone visits, with emphasis on reducing urgent care and ER visits, and adhering to ICARE values. Preferred experience includes working with diverse populations, particularly Veterans, Geriatric and High-Risk patients; 2-3 years NP experience preferred.
