Job Summary
Nurse Practitioner in Home-Based Primary Care (HBPC) for Veterans Affairs. Provides advanced clinical practice in geriatrics and extended care, develops individualized care plans, leads care within an interdisciplinary team, orders/interprets labs and diagnostics, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals to specialty services, educates Veterans and families, and delivers care in the Veterans' homes (including VVC and telephone visits). Emphasizes population health, disease management, health promotion and prevention, and palliative care within the HBPC program, striving to maximize health outcomes and reduce urgent/emergency visits. Upholds ICARE values and participates in routine home visits and remote communications as part of comprehensive patient care.
Required Qualifications
- Licensed Nurse Practitioner with national certification, eligible to practice in Michigan (HBPC/GERIATRIC focus)
