Job Summary
Provides in-home primary care for Veterans within the Geriatrics and Extended Care/Home Based Primary Care program, delivering health-promotion, disease-management, and palliative care in the patient’s home. Conducts labs/orders and interprets diagnostic studies, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals to specialty services, and educates patients and families. Works collaboratively with an interdisciplinary team to adjust plans of care to optimize outcomes, delivering care via home visits, Veteran Video Connection visits, and telephone visits while upholding ICARE core values (Integrity, Commitment, Advocacy, Respect, Excellence).
Required Qualifications
- Licensed independent provider with advanced education and national certification (Nurse Practitioner)
- Experience working with diverse populations a plus
- Prefer 2-3 years Nurse Practitioner Experience
