Job Summary
Nurse Practitioner in the Home-Based Primary Care/Geriatrics and Extended Care Service delivering a full range of clinical duties in patients' homes, including population-health planning, leadership within an interdisciplinary team, ordering and interpreting labs and diagnostics, prescribing interventions, making referrals to specialty services, and providing education to patients and families. Builds and adjusts individualized care plans, aims for optimal outcomes, and delivers care in the Veterans Affairs HBPC program through home visits, VVC visits, and phone consultations, while upholding ICARE values and focusing on geriatric and palliative care within the home setting.
