Job Summary
The Home-Based Primary Care Nurse Practitioner delivers comprehensive geriatric and extended-care services in patients' homes, developing individualized care plans, leading interdisciplinary teams, ordering and interpreting labs and diagnostics, prescribing interventions, initiating referrals, and educating patients and families. Responsibilities include home visits and VVC/telephone visits, managing care to maximize health outcomes, and aligning with ICARE values while supporting primary, geriatric, and palliative care within the HBPC program for Veterans with complex needs.
Required Qualifications
- Nurse Practitioner license
- National certification
- Licensed independent practitioner
- prefer 2-3 years NP experience (Geratrics/High-Risk populations)
