Job Summary
Home-Based Primary Care Nurse Practitioner delivers comprehensive geriatric and extended-care services in Veterans Affairs HBPC programs, including health promotion, disease management, primary and palliative care delivered in patients' homes. Responsibilities include developing individualized care plans, leading interdisciplinary teams, ordering and interpreting labs and diagnostics, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals to specialty services, providing patient and family education, adjusting plans of care to optimize outcomes, and delivering care via home visits, Veteran Video Connection, and telephone visits within the HBPC program; aligns with ICARE core values (Integrity, Commitment, Advocacy, Respect, Excellence).
Required Qualifications
- Nurse Practitioner license in state
- National certification as Nurse Practitioner
- Ability to provide care in home-based setting
- Experience with geriatrics and high-risk populations preferred
