Job Summary
Home-Based Primary Care Nurse Practitioner develops individualized plans for optimal population health and disease management in geriatrics/extended care, leads care within the HBPC program including health promotion and palliative care, conducts home visits and telehealth (VVC/telephone), orders/interprets labs and diagnostics, prescribes pharmacologic/non-pharmacologic interventions, initiates referrals, and provides patient/family education while collaborating with an interdisciplinary team and upholding ICARE values.
Required Qualifications
- Nurse Practitioner licensure
- national certification
- advanced education and training appropriate for NP scope of practice
- ability to provide care within geriatrics/extended care population
- experience with home-based care preferred (2-3 years)
