Job Summary
Home-Based Primary Care Nurse Practitioner providing geriatrics-focused, in-home primary and palliative care within the VA HBPC program. Delivers individualized care plans, leads and collaborates with an interdisciplinary team, orders and interprets labs and diagnostics, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals to specialty services, and educates patients and families. Performs home visits, Veteran Video Connection visits, and telephone follow-ups to Veterans with complex needs, aiming to maximize health outcomes and reduce urgent care/ER visits, while upholding ICARE values of Integrity, Commitment, Advocacy, Respect, and Excellence.
Required Qualifications
- Licensed Nurse Practitioner in Indiana
- National NP certification
