Job Summary
Provides full-scope advanced clinical care as a Home-Based Primary Care Nurse Practitioner for geriatrics, including health promotion, disease management, and palliative care in the home. Demonstrates leadership within the interdisciplinary team, orders and interprets labs and diagnostics, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals to specialty services, and delivers patient and family education. Delivers care through home visits, Veteran Video Connection (VVC) visits, and telephone visits to Veterans in the HBPC program, aligning with ICARE values (Integrity, Commitment, Advocacy, Respect, Excellence).
Required Qualifications
- Nurse Practitioner license
- national certification as Nurse Practitioner
- independent practice authority
- ability to provide care in home-based setting
- experience with geriatrics or high-risk populations (preferred)
