Job Summary
Home-Based Primary Care Nurse Practitioner provides in-home and remote (VVC/telephone) primary care for Veterans in Geriatrics and Extended Care. Develops individualized care plans to optimize population health and disease management, orders and interprets labs and tests, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals to specialty services, and delivers education to patients and families. Works collaboratively with an interdisciplinary team, delivers care across home visits, VVC video visits, and telephone visits, and prioritizes health promotion, geriatrics, primary care, and palliative care within the HBPC program while upholding ICARE values. NP practices to the full extent of education, training, and certification, with preferred 2-3 years of NP experience in diverse populations including Veterans and high-risk patients.
Required Qualifications
- Nurse Practitioner license
- National certification
- Advanced education (MSN or higher)
- Ability to practice as NP in Missouri
- Licensure to provide independent clinical practice
Desired Qualifications
- 2-3 years NP experience with diverse populations, Veterans, Geriatric and High-Risk patients
- Leadership and collaboration within interdisciplinary teams
- Experience in health promotion, disease management, and palliative care preferred
