Job Summary
Provides advanced clinical care as a Home-Based Primary Care Nurse Practitioner within the Geriatrics and Extended Care Service, delivering care to Veterans in their homes including Veteran Video Connection visits and telephone visits. Develops individualized plans to optimize population health and disease management, orders and interprets labs and diagnostics, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals to specialty services, and educates patients and families. Collaborates with an interdisciplinary team, adjusts plans as needed to achieve outcomes, and upholds ICARE values (Integrity, Commitment, Advocacy, Respect, Excellence). Focuses on health promotion, disease prevention, geriatric and primary care, and palliative care for Veterans with complex needs. Preferred experience with diverse Veteran populations, geriatrics, and high-risk patients; 2-3 years NP experience encouraged.
Required Qualifications
- Nurse Practitioner license
- national certification as Nurse Practitioner
- ability to practice as independent provider in home-based primary care
- experience with geriatrics or extended care preferred
