Job Summary
Provides home-based primary care for geriatrics within the GEC/HBPC program, delivering comprehensive care in veterans’ homes. Performs assessments, orders and interprets labs/tests, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals to specialty services, and delivers patient/family education. Collaborates with the interdisciplinary team, leads care planning to optimize population health and disease management, and enhances care quality while delivering home visits, Veteran Video Connection (VVC) visits, and telephone check-ins to complex patients. Upholds ICARE values (Integrity, Commitment, Advocacy, Respect, Excellence) and supports health promotion, prevention, and palliative care as part of comprehensive primary care.
Required Qualifications
- Nurse Practitioner license
- National certification as Nurse Practitioner
- Ability to practice independently
- Experience in geriatrics, Veterans health, or high-risk populations preferred (2-3 years)
