Job Summary
Nurse Practitioner delivers Home-Based Primary Care for Veterans in the Geriatrics/Extended Care program, conducting home visits and VVC visits, ordering labs and diagnostics, and prescribing pharmacologic and non-pharmacologic interventions. Collaborates with an interdisciplinary team, provides patient and family education, and adjusts plans of care to optimize health outcomes, anchored in ICARE values and in-home, comprehensive primary and geriatric/palliative care for Veterans with complex needs. Locations include Las Vegas, NV.
Required Qualifications
- Licensed Nurse Practitioner
- National certification as Nurse Practitioner
- Current state APRN license
