Job Summary
Provides advanced clinical practice as a licensed Nurse Practitioner in Geriatrics and Extended Care within the VA Home-Based Primary Care program. Develops individualized plans to achieve optimal population health and disease management; demonstrates leadership and collaborates with an interdisciplinary team; orders and interprets labs and diagnostic studies; prescribes pharmacologic and non-pharmacologic interventions; initiates referrals to specialty services; educates patients and families; assesses and adjusts care plans to optimize outcomes; delivers care in Veterans’ homes including health promotion, prevention, geriatric and primary care, and palliative care; conducts home visits, Veteran Video Connection (VVC) visits, and telephone visits; upholds ICARE values in clinical practice; preferred experience with diverse Veteran and geriatric/high-risk populations.
Required Qualifications
- Nurse Practitioner license
- national board certification
- eligibility/credentialing to practice as a NP in the VA system (Geriatrics/Extended Care)
- ability to provide care in home-based settings (HBPC) and telehealth modalities (VVC/telephone)
