Job Summary
Home-Based Primary Care Nurse Practitioner delivering geriatric and extended-care services to Veterans in the HBPC program. Leads individualized care plans, collaborates with interdisciplinary team, orders/interprets labs and diagnostics, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals to specialty services, and provides education to patients and families. Practices to full extent of education and certification in home-based settings, including home visits and VVC/telephone visits, with a focus on health promotion, disease management, and reducing urgent care needs. Prioritizes integrity, commitment, advocacy, respect, and excellence while delivering competent care to Veterans with complex needs in home settings. Preferred experience includes 2-3 years NP experience with diverse patient populations including Veterans and geriatric/high-risk patients.
Required Qualifications
- Nurse Practitioner license
- Board certification in an appropriate specialty (e.g., geriatrics)
- Independent practice with national certification
- Clinical privileges aligned with HBPC program
