Job Summary
Provides advanced clinical care as a Nurse Practitioner within the Geriatrics Extended Care/Home Based Primary Care program, delivering home-based geriatric primary and palliative care, health promotion and disease management. Exercises independent clinical practice, orders and interprets labs/tests, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals to specialty services, and educates patients and families. Delivers care through home visits, Veteran Video Connection (VVC) visits, and telephone visits to Veterans with complex needs, coordinating with an interdisciplinary team and upholding ICARE values of Integrity, Commitment, Advocacy, Respect, and Excellence.
