The Home-Based Primary Care Nurse Practitioner delivers geriatric and primary care in Veterans Affairs HBPC program, focusing on individualized care plans, leadership within the interdisciplinary team, and care delivery in the home and via VVC/telephone visits. Required Qualifications

  • Licensed Nurse Practitioner in Virginia
  • National certification as Nurse Practitioner
  • Ability to provide advanced clinical care in geriatrics and home-based primary care
  • Eligibility to provide care within Veterans Affairs system
  • Excellent collaboration with interdisciplinary team
  • Strong communication and patient-family education skills
