Job Summary
Home-Based Primary Care Nurse Practitioner provides comprehensive geriatric and primary care in Veterans Affairs HBPC program. Delivers evidence-based care in patients’ homes, conducts home visits and VVC/telephone visits, orders and interprets labs and diagnostics, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals, educates patients and families, and collaborates with an interdisciplinary team to optimize population health and reduce urgent care utilization. Practices to the full extent of education, training, and certification, upholding ICARE values, and contributing to health promotion, disease management, and palliative care within the GEC/HBPC program.
Required Qualifications
- Licensed Nurse Practitioner (NP) in good standing
- National certification for NP practice
- Ability to provide care within Home-Based Primary Care/GERIATRICS in VA system
- Independent practice authority as NP
- Experience or ability to perform comprehensive primary and geriatric care
- Proven ability to develop care plans and coordinate with interdisciplinary team
