Job Summary
Provides evidence-based primary care in the Home-Based Primary Care/Geriatrics and Extended Care program for Veterans, delivering care in patients' homes with emphasis on health promotion, disease management, and palliative care. Responsibilities include developing individualized plans, leading and collaborating with the interdisciplinary team, ordering and interpreting labs and diagnostic studies, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals to specialty services, and educating patients and families. Care is delivered through home visits, Veterans Video Connection (VVC) visits, and telephone visits, with attention to quality, timeliness, and adherence to ICARE values (Integrity, Commitment, Advocacy, Respect, and Excellence). Preferred experience includes 2–3 years as a Nurse Practitioner and serving diverse veteran and geriatric/high-risk populations.
Required Qualifications
- Licensed Nurse Practitioner
- National certification
- Practices within scope of education and certification
- Experience in geriatrics or extended care preferred
- Ability to provide home-based and video/telephone visits
- Demonstrated leadership and collaboration with interdisciplinary teams
