Home-Based Primary Care Nurse Practitioner

The Home-Based Primary Care Nurse Practitioner delivers comprehensive, evidence-based care for Veterans in the GEC/Home Based Primary Care Program, including geriatric and palliative care, via in-home visits, VVC, and telephone visits. Responsibilities include developing/implementing individualized care plans, leading within an interdisciplinary team, ordering/interpreting labs and diagnostics, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals to specialty services, educating patients and families, adjusting care plans to achieve optimal outcomes, and upholding ICARE values. The NP practices to the full extent of education, training, and certification, and provides care across health-promotion, prevention, and chronic disease management for a complex patient population in the home setting, with a focus on reducing unnecessary urgent care and ER visits. Preferred experience includes caring for a diverse patient population, Veterans, geriatrics, and high-risk patients; 2-3 years NP experience is preferred.
