Job Summary
Nurse Practitioner provides advanced clinical care in the Home-Based Primary Care Program for Veterans, delivering health promotion, disease management, and palliative care to complex geriatric patients in patients' homes. Core duties include developing individualized care plans, leading interdisciplinary collaboration, ordering and interpreting labs and diagnostic tests, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals to specialty services, delivering patient and family education, and adjusting plans based on patient outcomes. Responsibilities emphasize high-quality, timely care aligned with ICARE values, including home visits, Veteran Video Connection (VVC) visits, and telephone follow-ups to monitor and optimize health outcomes for veterans with complex needs.
Required Qualifications
- Nurse Practitioner license (APRN/NP)
- National certification as NP (e.g., ANCC)
- State licensure in North Carolina or eligibility
- Ability to provide care in home-based setting with geriatrics focus
- Experience with population health, disease management, and palliative care preferred
