Job Summary
Home-Based Primary Care Nurse Practitioner providing advanced clinical care in Geriatrics/Extended Care for Veterans in the HBPC program. Responsibilities include developing individualized plans for population health and disease management, leading and collaborating with interdisciplinary teams, ordering/interpreting labs and diagnostic studies, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals to specialty services, delivering patient and family education, and adapting plans to optimize outcomes. Delivers competent, home-based care to Veterans with complex needs, including health promotion, health prevention, geriatric and palliative care, and utilizes home visits, Veteran Video Connection (VVC), and telephone visits. Upholds ICARE values and performs other duties as assigned.
Required Qualifications
- Licensed Nurse Practitioner (NP) with current state license
- National certification as a Nurse Practitioner
- Ability to provide advanced clinical practice in geriatrics/home-based primary care
- Experience with health promotion, disease management, and palliative care in a home-based setting (preferred: 2-3 years)
- Demonstrated ability to order and interpret labs/tests, provide education to patients and families
- Strong collaboration skills with interdisciplinary teams
