Job Summary
Nurse Practitioner in the Home-Based Primary Care/Geriatrics and Extended Care program providing advanced clinical care in Veterans Affairs. Develops individualized care plans, leads and collaborates with an interdisciplinary team, orders and interprets labs/tests, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals to specialty services, educates patients and families, and adjusts plans to achieve optimal outcomes. Delivers care through home visits, VVC, and telephone visits to Veterans with complex needs, supporting health promotion, prevention, geriatric and palliative care within the HBPC program, and upholds ICARE values.
Required Qualifications
- Nurse Practitioner license in California
- National certification for Nurse Practitioners
- -independently practicing NP
- licensure to provide advanced clinical practice in geriatrics/home-based primary care
