Job Summary
Home-Based Primary Care Nurse Practitioner delivers in-home, evidence-based primary and geriatric/palliative care to Veterans within the HBPC program. Responsibilities include developing individualized plans to optimize population health and disease management, leading and collaborating with an interdisciplinary team, ordering and interpreting labs/tests, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals to specialty services, and educating patients and families. Provides care across health promotion, prevention, geriatric and primary care, and palliative care in home settings, including Veteran Video Connection (VVC) visits and telephone visits, with a focus on reducing urgent care and emergency visits. Emphasizes ICARE values and the full scope of NP education and certification within the VA system. Requires delivering competent care in the home of Veterans with complex needs and may involve home visits and telehealth.
Required Qualifications
- Licensure as a Nurse Practitioner
- National certification as Nurse Practitioner
- Experience in geriatrics or home-based care preferred
