Job Summary
Home-Based Primary Care Nurse Practitioner delivering geriatric and extended care within the VA HBPC program. Develops individualized plans for population health and disease management; leads and collaborates with an interdisciplinary team; orders and interprets labs/tests; prescribes pharmacologic and non-pharmacologic interventions; initiates referrals to specialty services; provides patient and family education; and delivers care in the home of Veterans with complex needs, including VVC and telephone visits. Emphasizes health promotion, health prevention, and reducing urgent care/ER visits in alignment with veterans’ goals of care. Preferred: 2-3 years NP experience with diverse patient populations.
Required Qualifications
- Licensed Nurse Practitioner
- National certification
- Provides care in geriatrics/primary care setting
- Ability to deliver home-based care to veterans
- Experience with diverse patient populations preferred (veterans, geriatrics, high-risk)
