Job Summary
Home-Based Primary Care Nurse Practitioner delivers geriatrics-focused care to Veterans in the HBPC program. Responsibilities include developing individualized care plans to optimize population health and disease management, leading and collaborating with the interdisciplinary team, ordering and interpreting labs and diagnostics, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals to specialty services, providing patient and family education, and adjusting care plans to achieve optimal outcomes. The role involves in-home and VVC/telephone visits within the GEC/HBPC program, care across health promotion, prevention, geriatric, primary, and palliative care, and adherence to ICARE values. Preferred experience includes working with diverse populations, Veterans, geriatrics, and high-risk patients (2–3 years NP experience).
Required Qualifications
- Nurse Practitioner license
- National certification as NP
- WA state NP licensure or eligibility
