Job Summary
Provides advanced clinical care as a Nurse Practitioner in the Home-Based Primary Care program for Veterans, delivering comprehensive geriatric and palliative care. Develops individualized care plans to optimize population health and disease management; leads and collaborates with an interdisciplinary team; orders/interprets labs and diagnostic studies; prescribes pharmacologic and non-pharmacologic interventions; initiates referrals to specialty services; educates Veterans and families; adjusts plan of care to improve outcomes; delivers care in the home and via Veterans Video Connection (VVC) and telephone visits; adheres to ICARE core values (Integrity, Commitment, Advocacy, Respect, Excellence).
Required Qualifications
- Nurse Practitioner license
