Job Summary
Home-Based Primary Care Nurse Practitioner delivering geriatric and extended care in Veterans Affairs HBPC program. Provides full-scope NP care including health promotion, disease management, care planning, and coordination with an interdisciplinary team. Conducts home visits and Veterans Video Connection (VVC) visits, orders and interprets labs and diagnostics, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals to specialty services, and educates patients and families. Practices to the full extent of education, training, and certification with emphasis on quality, timeliness, and Veteran-focused care within the GEC/HBPC program. Strong emphasis on care for veterans with complex needs, aligns with ICARE core values (Integrity, Commitment, Advocacy, Respect, Excellence). Preferred experience with diverse populations, Veterans, geriatrics, and high-risk patients; 2-3 years NP experience recommended.
Required Qualifications
- Nurse Practitioner, licensed independent provider
- Advanced education and national certification
- Experience preferred with diverse populations, Veterans, Geriatrics, High-Risk patients
- 2-3 years NP experience preferred
