Job Summary
Nurse Practitioner delivering Home-Based Primary Care for geriatrics and extended care within the VA. Provides evidence-based primary and palliative care in patients' homes, develops individualized care plans, orders and interprets labs, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals to specialty services, and delivers education to patients and families. Works collaboratively with an interdisciplinary team, supports health promotion and disease management, and performs home visits, Veteran Video Connection visits, and telephone visits to veterans with complex needs, aligning care with ICARE values.
Required Qualifications
- Nurse Practitioner license
- National certification as Nurse Practitioner
- State licensure as NP
