Job Summary
Home-Based Primary Care Nurse Practitioner providing comprehensive geriatric and extended-care services in Veterans Affairs HBPC program. Delivers evidence-based, patient-centered primary and palliative care in the home, develops and implements individualized care plans, leads and collaborates with the interdisciplinary team, orders and interprets labs and diagnostics, prescribes medications and non-pharmacologic interventions, initiates referrals to specialty services, and educates patients and families. Requires home visits, Veteran Video Connection visits, and telephone visits to Veterans with complex needs, aiming to maximize health outcomes and reduce unnecessary urgent care or ER visits, in alignment with ICARE values.
Required Qualifications
- Nurse Practitioner license
- Advanced practice registered nurse (APRN) with national certification in primary care/geriatrics
- Experience in geriatrics and high-risk patient populations preferred
- Ability to perform home visits, VVC visits, and telephone visits
- Develop and manage individualized care plans
- Order and interpret labs, tests, and diagnostics
- Prescribe pharmacologic and non-pharmacologic interventions
- Coordinate referrals and consults with specialty services
- Provide patient and family education
- Collaborate with interdisciplinary team
- Demonstrate ICARE values (Integrity, Commitment, Advocacy, Respect, Excellence)
- 2-3 years NP experience preferred
