Job Summary
Nurse Practitioner provides geriatric and extended care in the Home-Based Primary Care (HBPC) program, delivering comprehensive clinical care in patients' homes. Core duties include developing and implementing individualized plans to optimize population health and disease management, leading and collaborating with an interdisciplinary team, ordering and interpreting labs and diagnostic studies, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals to specialty services, and educating patients and families. The role supports care across home-based geriatric, primary, and palliative care services, including Veteran Video Connection (VVC) and telephone visits, with focus on reducing urgent visits and aligning with patients' goals of care. Demonstrates ICARE values—Integrity, Commitment, Advocacy, Respect, and Excellence—and may prefer 2-3 years of NP experience with diverse patient populations including Veterans and high-risk patients. Other duties as assigned.

- Licensed independent practitioner
- Advanced education (master's degree or higher)
