Home-Based Primary Care Nurse Practitioner provides advanced clinical care in geriatrics/extended care for Veterans, delivering health promotion, disease management, and palliative care through home visits, VVC, and telephone visits; leads in collaboration with interdisciplinary team, orders/ interprets labs and tests, prescribes interventions, initiates referrals, and educates patients and families; adheres to ICARE values and supports comprehensive home-based care for Veterans with complex needs.
