Job Summary

Home-Based Primary Care Nurse Practitioner provides geriatric primary, preventive, and palliative care to Veterans in the HBPC program. Delivers care in the home, via Veterans Video Connection (VVC), and telephone visits. Develops individualized care plans, orders and interprets labs and diagnostics, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals, and educates patients and families. Leads care coordination with an interdisciplinary team to optimize health outcomes, promote health, reduce urgent care visits, and uphold ICARE values (Integrity, Commitment, Advocacy, Respect, Excellence). Performs comprehensive assessments, adjusts plans as needed, and delivers care across primary, geriatric, and palliative domains for veterans with complex needs. Requires advanced education and national certification for an independent practice as a Nurse Practitioner; experience with diverse populations, especially Veterans and high-risk patients, is preferred.
