Job Summary
Provides advanced clinical care as a Nurse Practitioner in the Home-Based Primary Care (HBPC) program for Veterans in York, PA, delivering geriatric and palliative care in patients' homes. Leads population-health and disease-management efforts, collaborates with an interdisciplinary team, orders and interprets labs and diagnostic studies, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals to specialty services, and educates patients and families. Delivers care through home visits, Veteran Video Connection (VVC) visits, and telephone visits, aiming to maximize health capabilities and reduce unnecessary urgent care and ER visits, in alignment with ICARE core values. Prioritizes health promotion, preventive care, and complex needs management within the GEC/HBPC program.
