Job Summary
Nurse Practitioner in Geriatrics/Extended Care delivering home-based primary care to Veterans in Miami. Develops individualized plans to achieve optimal population health and disease management, demonstrates leadership, and collaborates with an interdisciplinary team. Orders and interprets labs, tests, and diagnostic studies; prescribes pharmacologic and non-pharmacologic interventions; initiates referrals; provides education to patients and families; adjusts care plans to optimize outcomes. Delivers care in the Home-Based Primary Care program including health promotion, health prevention, geriatric and primary care, and palliative care. Performs home visits, Veteran Video Connection (VVC) visits, and telephone visits to Veterans in HBPC; upholds ICARE values (Integrity, Commitment, Advocacy, Respect, Excellence). Preferred experience includes working with diverse populations, Veterans, geriatrics, and high-risk patients; 2–3 years NP experience preferred.
