Job Summary
Nurse Practitioner provides home-based primary care for Veterans in the Geriatrics/Extended Care Service, delivering comprehensive, evidence-based care in the patients' homes with a focus on health promotion, disease management, and palliative care. The role includes developing individualized care plans, leading interdisciplinary collaboration, ordering and interpreting labs and diagnostic studies, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals to specialty services, delivering patient and family education, and conducting home visits, VVC visits, and telephone contacts to Veterans in the HBPC program. Emphasizes professional leadership, adherence to ICARE values, and care across primary, geriatric, and home-based settings for patients with complex needs. Preferred experience includes working with diverse Veteran populations and higher-risk geriatric patients; 2-3 years NP experience is noted as preferred.
