Job Summary
Home-Based Primary Care Nurse Practitioner providing comprehensive geriatric and extended care in Veterans Affairs HBPC program. Develops individualized plans for population health and disease management, orders and interprets labs and diagnostics, prescribes pharmacologic and non-pharmacologic interventions, initiates referrals to specialty services, and educates patients and families. Delivers care through home visits, Veteran Video Connection (VVC) visits, and telephone visits to Veterans with complex needs, aiming to maximize health and reduce unnecessary urgent care and ED visits. Works collaboratively with an interdisciplinary team, upholds ICARE core values, and delivers care across health promotion, prevention, geriatric, primary, and palliative care in the HBPC program.
Required Qualifications
- Nurse Practitioner licensure
- Advanced practice certification
- Experience with geriatrics and high-risk populations preferred
- Ability to provide care in home setting
- Interdisciplinary collaboration skills
