Job Summary

The Nurse Practitioner provides comprehensive home-based primary care for Veterans within the GEC/HBPC program, delivering patient- and family-centered care in the home, including health promotion, disease management, geriatrics and palliative care, with home visits and VVC/telephone visits. Core duties include developing individualized care plans, leadership within the interdisciplinary team, ordering and interpreting labs and diagnostics, prescribing pharmacologic and non-pharmacologic interventions, initiating referrals to specialty services, educating patients and families, adjusting care plans to optimize outcomes, and upholding ICARE values. Preferred experience includes working with diverse populations, Veterans, geriatrics, and high-risk patients, with 2–3 years NP experience.

Required Qualifications

  • Licensed Nurse Practitioner in WA
  • National certification for NP
  • Ability to provide care in home-based primary care
  • Independent practice within scope of license
