Job Summary
Provides primary care in home-based settings for a geriatric patient panel, assessing, diagnosing, and treating chronic conditions, acute illnesses, and episodic medical needs within the scope of advanced nursing practice. Plans, directs, and evaluates patient care, performs selected diagnostic testing, develops treatment plans, prescribes medications, coordinates referrals, and performs procedures within licensure and protocols. Collaborates with supervising physicians, facility staff, and interdisciplinary care teams to deliver high-quality, patient-centered care across home-based settings. Requires MSN or higher-level Adult Nurse Practitioner, KY licensure (including DEA and controlled substances permits), and at least one year of full-time advanced nursing experience; on-call duties policy fulfillment; proficiency with EMR and related software; and a valid driver’s license. Schedule is Monday–Friday 8:00am–5:00pm with occasional weekend and weekday afterhours on-call; in-person care in Lexington, KY.
Required Qualifications
- MSN or higher-level Adult Nurse Practitioner required
- Licensure (or eligibility for licensure) for advanced nursing practice in KY, including DEA and controlled substance permits, required
- Minimum of one year of full-time advanced nursing experience required
- Previous experience in assisted living or home-based primary care and experience working with geriatric population preferred
- Fulfill requirements of on-call duty policies
- Proficiency using technology-based tools, including electronic medical records and other software programs
- Must be a licensed driver
