Job Summary

Home-Based Crisis Intervention (HBCI) Specialist provides in-home crisis intervention services to families and youth in crisis, conducts comprehensive assessments, tailors sessions to family needs, and promotes child, family, and community safety. Responsibilities include delivering short-term (up to 6 weeks), intensive interventions in clients’ homes, collaborating with parents and caregivers to set goals and develop service plans, teaching family members skills, advocating for concrete supports, coordinating with formal and informal community resources, and ensuring program reporting and continuous quality improvement. The role supports a hybrid schedule with in-home client work and remote administrative tasks, attends weekly team meetings onsite, and reports to the HBCI Team Lead. Education requirements include a Bachelor's or Master’s degree with relevant experience; licensure (LMSW/LCSW/LMHC/LMFT/LCAT) is preferred. Required Qualifications

  • Bachelor’s degree or Master’s Degree with 1-year relevant experience in community-based health or case management, or Master’s degree in Social Work or Mental Health Counseling.
  • LMSW, LCSW, LMHC, LMFT or LCAT licensure preferred.
  • Must have a valid NYS driver's license.
