Job Summary
Home and Community-Based Services (HCBS) Assessor conducts in-home assessments across the southeast New Mexico region (Roswell/SE NM) for adults in the Mi Via Waiver, using structured assessment tools and multiple systems to document daily living skills, behaviors, and supports; travels to participant homes, maintains confidentiality and HIPAA compliance, and collaborates with a team to support community-based services. Responsibilities include scheduling and coordinating annual in-home assessments, using electronic tools and templates to close cases, and ensuring timely documentation. Requires an Associate degree in a behavioral health field, 2 years of relevant experience, valid driver's license, and reliability for home-based work with travel. Strong written and verbal communication, computer proficiency, and optional Spanish bilingual skills are preferred.
Required Qualifications
- Associates degree in social work, psychology, other behavioral health science field
- 2 years of social service, nursing or other relevant work experience
- Valid, unrestricted driver license, proof of auto insurance and reliable transportation
- Ability to pass a Motor Vehicle Record check
