Job Summary
Coordinate access to medical, dental, and mental/behavioral services for children with complex conditions enrolled in Care4Kids. Develop comprehensive coordinated services plans, monitor progress, and integrate oversight of all services for foster care children and families. Assess client strengths and needs using trauma-informed principles, then arrange and facilitate Care4Kids services while documenting interventions to ensure quality standards are met. Update allergy and medication lists in Epic, maintain connections with child welfare agencies and community support systems, and lead team meetings to re-evaluate care plans. Manage goals and reporting processes to meet contractual obligations for the Department of Human Services and Children's Community Health Plan.
Required Qualifications
- Bachelor's Degree Social work, psychology, social services and other related fields
- 2+ years experience in health promotion, health advocacy, health education, clinical case management, child/family clinical social work, and community outreach or child welfare
- Experience with Microsoft Word, Access, Excel, and PowerPoint
- License Driver's License - State of Wisconsin
Desired Qualifications
- Master's Degree MSW
- Master's Degree APSW, LCSW, LPC
- Knowledge of Medicaid population and benefits
- Thorough understanding of the contract requirements related to the role of the Health Care Coordinator and the outcome measures for Care4Kids
- Bi-lingual
- Comfortable working in a fast-paced and changing environment
