Job Summary
Engage and re-engage clients who have lapsed in HIV care through comprehensive assessment, outreach, linkage, and re-engagement services. Conduct field operations to locate disengaged individuals, providing individual and group interventions focused on medication adherence, healthy living, and treatment access. Act as a liaison between counseling sites and medical clinics to ensure seamless enrollment for new clients, while delivering crisis interventions and supporting those not yet enrolled in clinic-based programs. Facilitate discussions on health lifestyle, relationships, and prevention strategies including PrEP and PEP. Collaborate with clinic team members to document interactions, contribute to program evaluation, and participate in case conferences. Maintain cultural and linguistic competency to serve diverse populations effectively.
Required Qualifications
- High school Diploma
- Valid Class C Drivers License
- proof of vehicle insurance
- reliable transportation for travel to a variety of sites throughout LAC
- Knowledge of HIV Disease
- behaviors that transmit HIV
- disease conditions that are co-morbid with HIV
- Ability to interact in a nonjudgmental and empathetic manner with PLWH and AIDS as well as their partners
- Must have good interpersonal skills
- Ability to advocate for clients
- Ability and interest in doing field-based work when necessary to locate or assist clients
- Must NOT be a current client of Contractor's clinic

- Demonstrates cultural and linguistic competency to effectively communicate with and support a diverse range of clients
