HIV Medical Case Manager

Job Summary
Assess new HIV patients for financial, psycho-social, and housing needs; develop comprehensive service care plans and evaluate Ryan White eligibility at intake and every six months. Work with patients to reduce barriers to care by arranging transportation, addressing social obstacles, and accompanying individuals to medical, legal, and housing appointments. Conduct intensive outreach visits on the streets, drop-in spaces, shelters, and patients' homes to connect with newly diagnosed or disconnected individuals. Participate in multidisciplinary HIV care team meetings to coordinate integrated care plans and exchange information within the partner notification program.
Required Qualifications
- Associate's or bachelor's degree in a related field and/ or equivalent two years relevant human services experience
- Experience working with people with substance use disorder
- Experience working with people experiencing homelessness
- Experience working with people experiencing incarceration
- Experience working with people experiencing sexual violence
- Commitment to harm reduction
- Knowledge of harm reduction strategies
- Trauma informed approaches
- Commitment to equitable and culturally appropriate care for a wide range of diverse populations
- Strong organizational skills

- Previous experiences conducting street level outreach with at-risk populations
- Knowledge of community resources, particularly addiction related resources and supports
