HIV Clinical Care Coordinator

Job Summary

Collaborate with multidisciplinary teams and external providers to coordinate care for high-acuity HIV patients, identifying and resolving barriers such as low health literacy, transportation issues, and language differences. Address patient inquiries via clinic visits, phone, and EMR messaging while facilitating rooming, intake, and vital sign collection. Perform point-of-care testing per CLIA guidelines, administer medications and immunizations, and document service units and patient data in the EMR. Complete quality controls, manage contaminated instrument transport, and monitor supply levels in exam rooms. Work Monday through Friday from 8 AM to 5 PM for 40 hours per week at the PHC University Campus in Des Moines.

Required Qualifications

  • High school diploma or equivalent.
  • One year of experience in a medical assistant, patient care technician, certified nurse assistant, phlebotomy, or related clinical position or completion of medical assisting program from an accredited institution.
  • Effective clinical skills related to the position essential functions.
  • Excellent customer service skills.
  • Effective verbal and written communication skills.
  • Excellent interpersonal, teamwork and collaboration skills with ability for a team approach to care plus develop and maintain effective working relationships with a diverse group of individuals.
  • Organization and prioritization skills.
  • Detail orientation.
  • Professional skills with ability to remain composed in all situations and show empathy.
  • Basic computer skills with proficiency using Microsoft Office applications and electronic health record system.
  • Effective problem-solving skills.
  • Current Basic Life Support (BLS) for Healthcare Providers.
  • Current Mandatory Reporter Training: Child Abuse and Dependent Adult Abuse recognized by the State of Iowa.
  • Must be available for Monday - Friday 8 AM - 5 PM.
  • Must be available for 40 hours per week.

Desired Qualifications

  • Work experience in a community health center or medical clinic.
  • Effective verbal and written communication skills in Spanish.
  • Phlebotomy skills.
