Job Summary

Conduct comprehensive needs assessments and schedule off-site home visits for clients living with HIV/AIDS. Develop individualized service plans addressing medical needs, coordinate enrollment in the 340B program, and make referrals for housing, transportation, and nutritional assistance. Monitor service effectiveness, facilitate counseling and support groups, and maintain HIPAA-compliant documentation. Submit monthly reports and attend ongoing training to ensure continuous job knowledge. This full-time role in Philadelphia reports to the Director of Clinical Services within a nonprofit serving underserved communities.

Required Qualifications

  • Master's degree in social work or related human service discipline
  • Bachelor's degree in social work or related human service discipline
  • Two (2) years case management and experience
  • Experience in working with persons living with HIV/AIDs
  • Demonstrated ability to communicate sensitively and clearly, orally and in writing
  • Ability to work as a part of a multi-disciplinary team
  • Ability to work under pressure
  • Willingness to travel throughout the city, including economically depressed areas
  • Ability to work autonomously yet responds to supervision
  • Superior oral and written communication, and organizational skills
  • Must be able to lift and carry up to 20 pounds occasionally
  • Requires the ability to communicate effectively with clients, staff, and external partners in person and by phone
  • May involve irregular hours, including occasional evenings or weekends, to meet client and program needs
