Job Summary
Provide physical therapy services to patients in their homes, collaborating with the team to develop comprehensive care plans. Deliver direct patient care while managing OASIS documentation and adhering to Massachusetts state licensing requirements. Maintain a 40-hour weekly schedule across five 8-hour shifts from Monday through Friday, with rotating weekends and holiday rotations as needed. This role requires two years of current home health PT experience and includes a $3000 weekly gross rate covering housing and travel stipends for eligible travelers.
Required Qualifications
- Reside beyond a 50-mile radius of the facility location
- Current Home Health PT experience: 2 years
- OASIS experience
- Certified Home Health PT experience
- Valid DL and Proof of Auto Insurance
- PT License for State of Massachusetts
- BLS Certification from AHA
- Home Health Certification – Relias or similar
