Hiring RN Hospice Case Manager in Lafayette, IN with $2360 Weekly Gross

Provide and coordinate care for assigned Hospice patients in a home setting, facilitating case management, admissions, and revisits. Develop and implement nursing care plans while empowering patients and families to reach healthcare goals through disease management education. Ensure physicians' orders are transcribed appropriately and assess patient status on an ongoing basis to document pertinent information. Notify physicians of significant status changes and complete all required charting. Requires a Registered Nurse with 2 years of experience and 1 year of recent hospice home care experience, including Home Care Home Base charting proficiency. Full-time, 40-hour day shift (8 a.m. to 5 p.m.) with $2,240–$2,360 weekly gross pay, including travel/housing stipends and mileage reimbursement.
