Provide and coordinate care for assigned patients in the comfort of their homes within Madison and Delaware counties. Develop and implement nursing care plans, including Start of Care and admissions, while supporting patient recovery and enabling them to spend quality time with loved ones. Empower patients and families by educating them on disease management and providing clinical oversight to interdisciplinary teams. Ensure physicians' orders are legible and transcribed appropriately, assess patient status on an ongoing basis, and document all visits within 24 hours. This 13-week contract requires a Registered Nurse with 2 years of experience, OASIS and HCHB charting proficiency, and a reliable vehicle.
